Transition is the process of moving from a pediatric to an adult model of care and is a high-risk period for adolescents and young adults (AYA) with rheumatic disease. This session will review the current state of transition in rheumatology and challenges faced by patients and providers. Recent evidence incorporating patient perspective on transition will be discussed. The session will conclude with discussing evidence-informed methods to provide developmentally response care for both pediatric and adult rheumatology practices.
