Achieving health equity and eliminating health disparities is a primary goal of Healthy People 2030. To accomplish this, it is necessary to address all determinants of health disparities, including social determinants of health (SDOH). This session aims to increase providers' knowledge and understanding of SDOH and how they impact patient health outcomes, identify organizational strategies that are aligned with federal laws and best practice guidelines, and discuss how data can be used to explore and address health disparities to enhance quality of patient care using the electronic medical record. Participants will learn skills and strategies to assess for health-related social needs and standardized screening tools within the clinical assessment.
